You may request a copy of your therapy records. Fees may apply for copying and mailing.

2. Right to Request Amendments

If you believe your record contains incorrect or incomplete information, you may request a correction.

3. Right to Request Confidential Communications

You may request that we contact you through a specific method (e.g., phone, email, mail) or at a specific location.

4. Right to Request Restrictions on Use & Disclosure

You may request limits on how we use or share your PHI. While we will consider your request, we may not always be able to comply.

5. Right to an Accounting of Disclosures

You may request a list of instances where we have disclosed your PHI outside of treatment, payment, or healthcare operations.

6. Right to File a Complaint

If you believe your privacy rights have been violated, you may:

Phone Calls and Texting

We may use phone calls and text messages to communicate with you regarding appointment reminders, scheduling, treatment updates, or other necessary information related to your care. By providing your phone number, you consent to receive such communications.

  • Confidentiality: While we take precautions to protect your privacy, phone calls and text messages are not always secure. If you prefer not to receive text messages or calls regarding your care, please notify our office.

  • Emergency Communication: We do not use text messaging for emergencies. If you are in crisis, please call 911 or go to the nearest emergency room.

  • No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties

  • Opting Out: You have the right to opt out of text message communication at any time. To do so, reply “STOP” to any text message or contact our office directly.
